BILLING PROCESS

Your Healthcare Provider has determined that one of our services are medically necessary for your care. Your provider will complete a requisition form (script) and forward the completed script with insurance information to Convergent Diagnostics Laboratories, LLC and its billing company affiliates.

Patients with insurance plans will receive an explanation of benefits (EOB) explaining what laboratory services were provided and the amount charged to the insurance company. However, this is NOT a bill. If you have any questions about the billing process, please call (214) 501-5171 option 3 to speak with a member of our billing department.

Convergent Diagnostic Laboratories is committed to high-quality, affordable patient care. We are an in-network provider for laboratory and pathology services on all major medical plans. We are sensitive to the fact that some patients have no insurance or high-deductible insurance plans and we remain committed to maintaining very patient-friendly billing policies.
